Covid-19 Testing

Every patient scheduled for surgery at Park Ten Surgical Center MUST be tested for COVID-19, within 7 days of surgery. Tests conducted with Park Ten must be done 5-7 days depending on the surgery date to ensure the test results are back in time for surgery. Testing is conducted by appointmentonly. To schedule a COVID-19 test appointment with Park Ten, patients must follow the steps below. If you choose to have your test completed at an outside facility, please notify us of the name of the facility and the date you are testing by emailing covidtesting@parktensurgery.com.

The types of tests that are acceptable are PCR Test, Molecular Antigen Test and Antigen Fluorescent Immunoassay Test(all are nasopharyngeal swab tests). Antibody Tests are not acceptable. Results should be faxed to1-833-562-8894 or emailed to covidtesting@parktensurgery.com no later than noon, one business day prior to your surgery date.

Please understand, if we do not receive the results in time, your surgery will need to be rescheduled. Patients agree to self-quarantine and maintain social distancing from the time they are tested until the date of surgery. We understand Patients work/attend school and must visit their physician's office for preop and that is ok! However, we ask Patients to agree to remain self-quarantined and to adhere to all government standards regarding face coverings and social distancing from Testing Date to Surgery Date. This means wearing a facial covering, NO travel, minimizing outings, NO large gatherings and maintaining six feet of distance from others as much as possible.


Step 1: Go to www.onemedicalpassport.com and complete the patient registration and medical history for the upcoming surgery. This MUST be done BEFORE a COVID-19 testing appointment will be confirmed. If you do not have access to the internet or do not have anyone that can assist you with completing this step go straight to step 2.

Step 2: Send an email to covidtesting@parktensurgery.com to schedule your Pre-Surgical COVID-19 Test. Patients that do not have access to email should call 281-647-2333 and leave a voice mail message. Please include the following information:

  • Patient’s First and Last Name
  • Patient’s Date of Birth
  • Surgeon’s Name and Surgery Date
  • Contact Telephone Number
  • Email Address
  • A Preferred Appointment Time between 7:15am-12:30pm, Monday, Wednesday or Friday, 5-7 days prior to your surgery date

Once we have confirmed you have completed Step 1, you will receive an email confirmation for your COVID-19 testing appointment. We will schedule your appointment as close to your preferred time as possible.

On the day of testing, you MUST bring the following with you:

  • Patient’s Valid Government Issued Photo ID for all patients 18 years of age and older.
  • Patient’s Parent’s Valid Government Issued Photo ID for all patients less than 18 years of age.
  • Valid Insurance Card.
  • Writing Pen with which to sign paperwork.
  • Mask or face covering to wear while interacting with our staff.

*Patients arriving without a valid Government Issued Photo ID or insurance card as described above will NOT be tested.*


DO NOT pull into the parking lot. Instead, drive passed the flag poles to the end of the cul-de-sac towards the wooden fence. You will see a sign that reads, “Patient Pick Up”. Turn right into the driveway and park under the porte-cochere. You will see double glass doors to your right. Remain inside your vehicle and a tester will come out to you. Please have your face mask/covering on, your writing pen, your valid insurance card, and valid Government Issued Photo ID already out and available to give to the tester. It will take anywhere from 3-6 days, depending on the lab, to get results back. Please do not call inquiring about test results as we will contact you via email with your negative results. Only patients that test positive will receive a telephone call.
